Catheter ablation of atrial fibrillation versus antiarrhythmic therapy in patients with left ventricular systolic dysfunction

Aim. To evaluate the efficacy of catheter ablation (CA) atrial fibrillation (AF) in patients with left ventricular systolic dysfunction and effect procedure on course chronic heart failure (CHF) compared use antiarrhythmic therapy (AAT). Materials methods. The study included (n=80) ejection fraction (LVEF) less than 50% AF, who were divided into 2 groups: first group (n=40) underwent CA, second AAT. At baseline after 6 months follow-up, transthoracic echocardiography, determination NT-proBNP concentration, 6-minute walk test Minnesota Living With Heart Failure Questionnaire performed. Results. After absence documented AF paroxysms was observed 80% CA versus 45% AAT (p=0,001). A significant increase LV EF revealed both groups, however, it more (57% [50,5; 60] 46% [40; 50] (p=0,0001)). There a decrease ventricle ESD size volume group, while dynamics these parameters did not change within six (p=0.0001). An improvement clinical CHF level 196,6 [116,5; 327,1] pg/ml 540.5 [382,8; 861,2] (0,0001), distance increased to 470 m [400; 500] 368 [330; 400] (p=0,0001), respectively, functional class (1,15±0,67 2,05±0,55 (p=0,0001)) quality life (p=0,0001) observed. Conclusion . is an effective method for CHF. promotes reverse remodeling, NT-proBNP, as well exercise tolerance.
